W-2s   -   Have you moved??  Where will it go??  Where is it now??

Whenever your mailing address changes you will need to fill out an Employee Action Request Form (E.A.R.) to change your address with the state system or change it directly on-line on the my.csusm.edu Portal. Many people find out too late (especially around tax time) that their addresses were never changed causing important documents (like W-2s or benefits information) to be sent to the wrong address. Some of these documents do actually make their way back to the sender but most end up lost in the U.S. Mail system. Don't let this important little act cause you to be late in paying your taxes.

For those of you who supervise Student Assistants - they need to change their address with HREO (on a Student Payroll Action Request - S.P.A.R.). Many of them make the change with Admissions and Records but neglect to do so with HREO.

W-2s will be mailed to your home address by the State Controller's Office.  If you don't receive your W-2 by February 15th please contact Payroll as soon as possible, so that they may check to see if it has been returned to CSUSm as well as checking to make sure you have the correct address on file with us.  Duplicate W-2s must be requested directly from the State Controller's Office at a cost of $8.50 each.  They take approximately 2-4 weeks to be printed.
