Student athletes are required to complete specific forms and turn them in to the Athletics Department before they are allowed to participate in practice or competitions. The medical forms are available below; other necessary forms can be picked up from the Athletics Front Desk in the Clarke Field House. If you have any questions, please e-mail Todd Snedden, or call him at (760) 750-7109.

Medical Forms
Returning athletes, new students, and tryout participants each must fill out different forms.
All athletes (returning, new, and tryouts) must have a doctor complete and stamp the Pre-Participatory Physical Screening form.  Please refer to the Athletic Training section for these forms.
