Returning athletes, new students, and tryout participants each must fill out different forms. All athletes (returning, new, and tryouts) must have a doctor complete and stamp the Pre-Participatory Physical Screening form. Please visit the following link and select the appropriate form packet.

Sports Psychology Service
Sport consultation is available to all CSUSM
student-athletes. Walk-in consultations will be available weekly at the Clarke
Field House/University Student Union. Student athletes can sign up for these
walk-in hours as well as alternative individual consultation times by
contacting Erin Kelly Rounseville at: 760.650.4913.

Brian Smith - Head Athletic Trainer
Brian has been the Head Athletic Trainer at CSUSM since October 2007. He received his Bachelor’s of Science in Exercise Physiology from California State University, Chico in 2001, and from there went on to receive his Master of Science in Kinesiology with emphasis in athletic training from Indiana University in Bloomington in 2002. While at Indiana University, Brian was the graduate assistant athletic trainer with football and women’s rowing. Following graduate school, Brian worked as a one-year fellowship athletic trainer for the United States Olympic Committee both in Chula Vista, CA and Lake Placid, NY. He went on to become the Head Athletic Trainer at Rancho Buena Vista High School in Vista, CA for four years before taking the position here at CSUSM.
Brian has been a national certified athletic trainer through the NATA since August 2001, and is certified as an instructor in CPR and First Aid through the American Red Cross. He is a member of the National Athletic Trainers’ Association (NATA) and is heavily involved in Far West Athletic Trainers’ Association (FWATA) and California Athletic Trainers’ Association (CATA) where he sits on managing board as the CATA Region 6 Director.
Brian resides in Temecula with his wife Kathy.
