This is the place for new student-athletes, returning student-athletes, and tryout participants to download their paperwork. For tryout participants, please click the link directly below. For new and returning athletes, please find your sport then click the appropriate link to access your paperwork. All forms must be completed in full or they will not be accepted.

Forms can be faxed to (760) 750-3660 for initial screening. However, all original documents must be received by the Office of Athletics prior to any athletic participation clearance.

For Tryout Participants: We need the original copies prior to your tryout.

For New and Returning Student-Athletes: You will not be officially cleared to condition or practice until your original documents have been received and approved.
