This address change/emergency contact form is only for the use of the Office of the Dean of the College of Humanities, Arts, Behavioral and Social Sciences at Cal State University San Marcos. The information you complete and submit is not shared with any other campus department or external entity. The form is intended for faculty and staff in the College to ensure that up to date contact information is stored at the Dean's office level to provide proper communications in case of unusual circumstances and/or if an emergency may occur.
