Notice of Required Disclosures

Federal Regulations (Part 668.41 - Student Assistance General Provisions) mandate that a Notice of Required Disclosures be provided to all enrolled students on an annual basis and to all prospective students. Following is a list of the required disclosures, summary descriptions, and information to obtain a detailed copy of each disclosure.

Constitution and Citizenship Day

 In 2004, Congress established Constitution and Citizenship Day, a federal holiday commemorating the signing of the U.S. Constitution on September 17, 1787, and its ratification in 1788. For the last 232 years, Constitution Day has been held on September 17 to recognize the road this document has paved for not only the United States, but for other countries around the world.

Athletic Participation Rates & Financial Support Data

This disclosure provides information on the number and gender of full-time undergraduates participating in the intercollegiate athletic competitions and on each team. Also included is the information on coaches, coach salaries, and the revenues and expenses associated with each sport.

CSUSM submits data to the Office of Postsecondary Education (OPE) Athletics regarding our participation rates and other Equity in Athletics Institution data

Information regarding graduation rates and academic success rates for CSUSM student-athletes are included in the Federal Graduation Rates and Athletics Academic Success Rate Reports.

Campus Security

This disclosure provides a directory of personnel for the CSUSM Police Department, as well as statistics on campus safety and crime, policy and procedure for reporting crimes, policies concerning the security of and access to campus facilities, policies concerning campus law enforcement, campus safety and crime prevention programs (including drug and alcohol, sex offenses & education programs).

Copyright Infringement

The purpose of this document is to establish uniform policies and campus wide procedures that assure campus compliance with all state and federal laws applying to copyrighted material. This policy applies to all copyrighted materials that in anyway use campus-computing resource.

Family Education Rights and Privacy Act

This disclosure informs students of their rights and the University's policies concerning access to and privacy of student educational records. Information on FERPA is published in the 2019-2020 Catalog.

Financial Assistance

This disclosure provides information on all available federal, state, local, private, and institutional financial need-based and non-need-based assistance programs.

Student Health Services

Identity Theft

This disclosure certifies that the CSU, San Marcos Finical Aid Department has systems to identify and resolve discrepancies in information received from different sources including the use of false identities. Suspected violators will be referred to the Inspector General of the U.S. Department of Education.

Immigration Requirements for Licensure

The Personal Responsibility and Work Opportunity Reconciliation Act (PRAWORA) of 1996 (P.L. 104-193), also known as the Welfare Reform Act, includes provisions to eliminate eligibility for federal and state public benefits for certain categories of lawful immigrants as well as benefits for all undocumented immigrants.

Students who will require a professional or commercial license provided by a local, state, or federal government agency in order to engage in an occupation for which the CSU may be training them must meet the immigration requirements of the Personal Responsibility and Work Opportunity Reconciliation Act to achieve licensure. Information concerning the regulation of these requirements is available from the Associate Vice President of Enrollment Management Services, Cal State San Marcos 760-750-4826.

Institutional Information for CSUSM

This disclosure provides general information related to CSUSM including, but not limited to: cost of attendance, procedures to withdraw from school, refund policies, return of Title IV funds, academic programs, accrediting agencies, special services for disabled students, study abroad, and availability of employees responsible for the dissemination of this information.

Nondiscrimination

Placement Test Data

Test data for the English Placement Test (EPT) and the Entry Level Math Test (ELM) is reported by the CSUSM Office of Institutional Planning and Analysis.

Services to Students with Disabilities

Disability Support Services: Students with documented disabilities who will require reasonable accommodations to take part in University sponsored programs and activities will need to contact Disability Support Services at (760) 750-4905 or (760) 750-4909 (TTY) for assistance.

Social Security Number Use

The University uses the Social Security numbers of students and/or parents in accordance with federal and state regulations. Applicants are required to include their correct Social Security numbers in designated places on applications for admission persuant to the authority contained in Section 41201, Title 5, California Code of Regulations, and Section 6109 of the Interal Revenue Code (26 U.S.C 6109). The University uses the Social Security number to identify students and their records including identification for purposes of financial aid eligibility and disbursement and the repayment of financial aid and other debts payable to the institution.  Also, the Internal Revenue Service requires the University to file information returns that include the student's Social Security number and other information, such as the amount paid for qualified tuition, related expenses, and interest on educational loans. This information is used by the IRS to help determine whether a student, or a person claiming a student as a dependent, may take a credit or deduction to reduce federal income taxes.

Teacher Preparation Programs

Annual Report Card on California Teacher Preparation Programs for the Academic Year 2010-2011

This report, prepared by the California Commission on Teacher Credentialing (CTC) and released October 2012, is required by Title II of the 2008 Reauthorization of the Higher Education Act. It is the twelfth annual report of its kind and includes a description of the credential requirements to teach in California public schools. In addition, it includes qualitative and quantitative information on teacher preparation programs and pass-rate data for all examinations used by the state for credentialing purposes. 

Textbook Information

The CSUSM bookstore displays the following information for each class section: textbook title, the International Standards Book Number (ISBN), and the manufacturer's suggested retail price for required and recommended textbooks and supplemental materials.

Transfer Course Information

Transferable courses are those designated for baccalaureate credit by the regionally accredited college or university offering the courses and accepted as such by the campus to which the applicant seeks admission. Review the transfer credit policy for the number of units that can be transferred to CSUSM and meet degree requirements. 

Voter Registration

The Office of the Secretary of State and the University are committed to making voter information forms and online registration available.

Withdrawal from the Institution

Students withdrawing before or during the Late Registration Period of an academic term or semester should use my.csusm.edu
