Evaluation and Recommendation Request

To request an evaluation and recommendation for the Autism Spectrum Disorder added authorization (ASD), please go to the School of Education website and download the appropriate application following these instructions:
 
•  www.csusm.edu/education
•  Click on the link titled, "Students: New & Continuing" located on the left-hand side of the screen.
•  Next, click on the link titled, "Current SOE Students".
•  Click on the link titled, "Credential Finishing Information".
•  Under the heading "Credential Evaluation Request Forms" select the application that corresponds to the type of credential you already hold-Level 1 or 2.
•  Download the application and the Credential Evaluation Fee Payment Form. Complete and submit the application and proof of payment to the Student Services Center in University Hall 221. You may drop it off in person or mail it via regular mail.
 
Also, please note that if you have never attended CSU, San Marcos, we will need a copy of official transcripts- one verifying the Bachelor's degree and, if different, one from the institution where you received your initial teaching credential. We must have these on record as part of your file for accreditation purposes. If you are a graduate of CSUSM (BA or credential), you do not need to provide transcripts.

All requests for evaluation are processed in date order.
