2. The target population to be served by this project.

3. The goal: what you wish to change. The objectives: how you will attain that goal. The activities: what you will do to meet the objectives.

4. Evaluation: how you will know if you are successful, the anticipated outcomes and benefits.

5. What resources you have and what additional resources you will need to either pilot or implement this project.
