Regular Foundation Employees. Regular foundation employees are appointed

GROUP HEALTH INSURANCE (CalPERS Employer Code

The Cal State San Marcos Foundation shares the monthly premium cost
with its regular employees for health insurance. Regular employees are
those employees appointed to a position for a minimum of one (1) year,
working a minimum of twenty (20) hours per week. The Foundation has
contracted with the California Public Employee's Retirement System (CalPERS)

Cal State San Marcos Foundation contributes 100% of the monthly premium for
dental insurance for each regular employee and 100% of the monthly premium
for spouses and eligible dependents.

Cal State San Marcos Foundation contributes 100% of the monthly premium for
vision insurance for each regular employee and 100% of the monthly premium
for spouses and eligible dependents. Regular employees are also eligible
for Computer Vision Care through this benefit that allows employees to
obtain corrective eyewear designed to meet specific health and vision needs
of computer users.

Cal State San Marcos Foundation contributes 100% of the premium for Life
and Accidental Death and Dismemberment (AD&D) insurance for regular
employees. Both benefits are equal to one times your Annual Earnings up to
a maximum of $50,000. Employees are also eligible to select
Optional Life and AD&D insurance programs.

Cal State San Marcos Foundation contributes 100% of the premium for
Long-Term Disability insurance for regular employees. Benefits are 66 2/3%
of the first $7,500 of your Pre-disability Earnings, reduced by Deductible
Income.
LONG-TERM CARE (CalPERS)

Regular employees are eligible to participate in a Long-Term Care program
provided through CalPERS. This benefit allows employees to
establish benefits for their long term care needs and is voluntary.

Cal State San Marcos Foundation offers a defined contribution retirement
plan that operates under Section 403(b) of the Internal Revenue Code (IRC).
The Plan was established on September 1, 1997. The purpose of the Plan is
to provide retirement benefits for participating employees. Benefits are
provided through TIAA-CREF. Regular employees are eligible to participate
in the retirement plan on the first day of the month following date of
hire. When you begin participation in the Plan, contributions will be made
automatically to the funding vehicles that you've chosen. To read the

The contributions are based on a percentage of your compensation, according
to the schedule as follows:
Additionally, eligible employees are immediately 100%
vested within this plan.

Cal State San Marcos Foundation offers its regular employees choices
related to its benefit programs. The Cal State San Marcos Cafeteria Plan
qualifies as a cafeteria plan under Section 125 of the Internal Revenue
Service Code. Because of this plan, employees can choose from several tax

PERSONAL TIME OFF (PTO)
Regular Employees accrue PTO at the rate of six (6) hours per month (3
hours per pay period). PTO may be used for a personal illness, medical
appointments, school appointments, observance of religious holidays, or to
conduct personal business.
Note: Regular Employees that work less than forty (40) hours per week
qualify for PTO on a pro-rata basis.
HOLIDAY PAY (13 DAYS)
Employees who are regularly scheduled to work 40 hours per week and who
have been appointed for a period in excess of six months qualify for
Holiday Pay. Those holidays are: January 1st; Martin Luther King Jr.’s
Birthday; Lincoln’s Birthday; Washington’s Birthday; Cesar Chavez’s
Birthday, Memorial Day; July 4th; Labor Day; Admission Day; Columbus Day;
Veteran’s Day; Thanksgiving Day; December 25th; and other days designated
by the Governor as a public holiday. Please see the current Holiday
Schedule located on our website for further information.
Note: Regular Employees that work less than forty (40) hours per week
qualify for holiday pay on a pro-rata basis.
PERSONAL HOLIDAY
In addition to the 13 holidays observed by the Foundation, regular
employees receive Personal Holiday time each year. Personal Holiday time is
available for all eligible employees to use as of January 1st of each
year and must be used by December 31st. The Personal Holiday is capped
at a day and one half (1 1/2) each year and must be used in one full day
increment (8 hours) and one half day increment (4 hours).
Note: Regular Employees that work less than forty (40) hours per week
qualify for the personal holiday on a pro-rata basis.
EDUCATIONAL ASSISTANCE
The Foundation has established an Education Reimbursement Program which
assists regular employees who aspire to higher levels of skills and
professional competencies. Educational reimbursement may be used for
successful completion of approved work-related classes or accredited
courses leading to an undergraduate or graduate degree that supports the
mission of the Foundation. The Foundation will reimburse for fees at Cal
State University San Marcos or fees incurred at other institutions not to
exceed the equivalent Cal State University San Marcos mandatory fees for
zero to six units per semester. Transportation expense, parking fees,
books, lab fees and other related expenses are NOT covered.
To qualify, a regular employee must be employed by the Foundation for one
year and must apply before beginning the coursework. In addition, funds
must be available and approved by the Project Director well in advance.

EMPLOYEE ASSISTANCE PROGRAM (EAP) Horizon Health

The Employee Assistance Program provides confidential counseling and
referral services to all regular employees and members of their household
24 hours a day. This benefit is available through an external provider to
assist you and your family in dealing with a wide variety of problems or
concerns such as those involving relationships, parenting, drug/alcohol
abuse, depression, communication, stress, and concerns related to your job.
All services are kept strictly confidential.
WORKER’S COMPENSATION INSURANCE
All employees are covered by the Foundation’s Workers’ Compensation
Insurance. The cost of the insurance is borne by the Foundation with no
cost to the employee. The purpose of Workers’ Compensation is to assist
employees who have suffered a job-related injury or illness. If you are
injured on the job or incur a work-related illness, please contact your
supervisor and the Human Resources office immediately in addition to
seeking medical treatment with follow-up care as required.
STATE DISABILITY INSURANCE
All of our employees are covered under the State Disability Insurance plan
which provides benefits in the event you need to miss work due to a
non-work-related accident or illness. A small percentage of your wages will
be deducted each pay period for disability insurance. Benefits will begin
from the first day you are hospitalized or after the seventh day of illness
or accident if you are not hospitalized. You must file a claim with the
State to receive this benefit.
An employee must file a claim with the Employment Development Department to
receive any benefits. Claim forms for California State Disability benefits
can be requested from your nearest California Employment Development
Department.
PAID FAMILY LEAVE
All Foundation employees will be eligible to receive
paid family leave benefits from the State of California in the event they
are on an approved leave of absence to care for a seriously ill family
member or to bond with a new child. A small percentage of your wages will
be deducted each pay period for this paid family
leave benefit. You must file a claim with the State to receive this
benefit. Please contact Human Resources for additional information
related to this program.
UNEMPLOYMENT INSURANCE
All employees are covered by unemployment insurance which provides payment
to an eligible individual who is unemployed, physically able to work and
available to accept and actively seek employment as directed. The
Foundation pays the entire premium for this insurance for our employees.
JURY DUTY
It is the Foundation's policy to enable its employees to fulfill their
civic obligations. Regular Central Staff and Agency employees are
eligible to receive paid jury duty benefits for up to 5 days per year,
provided the employee has at least one-year of service with the
Foundation. All other employees are not eligible for paid jury duty
benefits but may serve on jury duty on an unpaid basis. Regular
employees working on grant, contract or gift funds and all temporary
employees are not eligible as well. Employees are required to
provide evidence of jury duty attendance in order to be eligible for
paid or unpaid time off.
BEREAVEMENT
When a death occurs in an employee's immediate family which necessitates
his/her losing regularly scheduled work time due to bereavement, Regular
and Temporary employees who are eligible for other paid time off
benefits (i.e., vacation and PTO) will be compensated for a maximum of
two days for such lost work. Immediate family member means
parents, spouse, spouse's parents, children, brothers, sisters, other
dependent family members, or other significant others living in the
employee's household.
TIME OFF FOR PARENTS
If you are a parent with one or more children in kindergarten, or in grades
1 through 12, you may take time off of up to 40 hours per year to attend
authorized school activities which involve one or more of your school-age
children.
SERVICE AWARDS
All employees are eligible to receive service awards that honor continuous
years of service with the Foundation starting at 5 years.
FITNESS CENTER
Regular Foundation employees are eligible to receive a discounted
membership to the M. Gordon Clarke Fieldhouse/University Student Union
Fitness Center located on the campus of Cal State San Marcos. The
discounted fee is available through payroll deduction and does require
that the employee maintain the membership a minimum of 90 days.
