Agreement Forms

In our efforts to provide you with the latest computer hardware and software tools, we ask that you please fill out and return the appropriate form filled in completely to the Faculty and Staff Help Desk.

Upgrade Agreements

Refresh Agreement Upgrade Form- This form is for anyone that is due a Upgraded Refresh Computer this Summer/Fall. To find out if you are due for a Refresh, please check the number from the Computer/Refresh tag on the front of your computer (i.e. 12, 13, etc).

• For school year 2007/2008, your Computer/Refresh tag number should be 14 or less.
• For school year 2008/2009, your Computer/Refresh tag number should be 15 or less.

Right To Use (RTU) Agreements

RTU with Network Use- This form is for anyone that would like to purchase a retired Refresh computer for on campus with Networking. See form for details.

RTU with No Network Use- This form is for anyone that would like to purchase a retired Refresh computer for on campus or off campus with No Networking. See form for details.

Microsoft Software Agreement

Microsoft Campus Agreement Form- This form is for Faculty and Staff PC users that are eligible to receive a copy of Microsoft products for work related use on their personal computer. Please review the list of Microsoft products that are available for home use.
