*Name of User		Name of User is a required field!
*Campus Email Address		Campus Email Address is a required field!
*Phone Number		Preferred contact number Phone Number is a required field!
*Location Where Hardware Was Used		If used at a home location. Enter "Home" in the field. Location Where Hardware Was Used is a required field!
*Comments		Please ask user why they are dropping off the hardware and enter it into the coment field. Comments is a required field!
*Hardware Type		Hardware Type is a required field!
*Asset Tag		If there is no asset tag on the hardware, enter "NONE" into the field. Asset Tag is a required field!
*Serial Number		Serial Number is a required field!
*Name of Computer		If computer does not have a name labeled on the computer, enter "NONE" into the field. Name of Computer is a required field!
Inventory		Any accessories which are returned with a computer. Inventory is a required field!
*Current Location of Hardware		Indicated where the items can be found. e.g., labeled bin and name on the label. Current Location of Hardware is a required field!
