Production Release Forms

These forms are designed to protect the University from liability as well as to ensure that the University can make use of materials produced using state equipment and staff labor.

• Standard release form - for people appearing in CSUSM media productions.
• Faculty/Guest Release Form - for Faculty and their guests appearing in CSUSM productions.
• Establishment Release Form - for facilities or buinesses which appear in CSUSM productions.
