2012 W-2 Address Updates

The end of the tax year is quickly approaching, which means W-2s will be issued soon. Please take a moment to verify your address listed at the bottom of your October 2012 paycheck dated 10/31/2012.  If the address on the October check is incorrect, please follow the online self-service instructions listed below. In order to ensure you receive your W-2 at the correct address, please update by December 12, 2012.

You can verify and update your address online.

Log in to my.csusm.edu:    https://my.csusm.edu/

Enter your:      User ID
Enter your:      Password
Click on:          My HR Information
Click on:         Personal Information

View your current home address
To update your home address:
Click On:         Change Home/Mailing address
Click On:          Edit
                     enter the change
Click On:         Save

This will update your official address and will automatically notify Payroll.

An alternative to updating your address online is completing and submitting a hardcopy of the form below to the Payroll Office:
