Due by no later than Feb. 1, 2013 to the School of Nursing. Submit all of
these (1- 5) as one complete packet by no later than the due date.

The School of Nursing cannot access electronically submitted transcripts.  If you have attended schools elsewhere, you need to have the official transcript (for the School of Nursing) sent to your home address. Do not open an Official, sealed transcript.  Collect these, and then submit along with the other required materials by no later than the deadline.

1. An official transcript for all college level coursework (from every separate college attended) through Fall 2012 (with the exception of CSU San Marcos coursework, if you are a current student at CSUSM -- We have access to all CSUSM coursework). If you have earned AP, IB, CLEP or Military course credit, send these official transcripts to the University's Office of Admissions and Recruitment, and we will obtain these scores/transcripts from that office (instructions on how to obtain these scores are linked within the Application Instructions). Be sure to indicate on your School of Nursing application that we should be looking for these AP, IB, CLEP or military transcripts. Transfer students must keep track of separate University Admissions submittal deadlines.
2. Submission of unofficial transcripts or Web printouts showing coursework in progress for Spring 2013, with the exception of CSUSM coursework.
3. If you completed any of your Science courses prior to 2002, and such courses will be applied towards meeting the requirements in the Pre-Nursing Core, you will need to submit Course Outlines from the Academic Year in which you completed such courses. All institutions maintain course outlines for courses offered there.
4. Submittal of two stamped business sized envelopes, addressed to yourself. Please use 'forever' stamps that will not require extra postage, if the postal rates are increased in the near future (envelopes should be approx. 4" x 9.5").
5. Submit a printed copy of your completed on-line School of Nursing Program application.
6. If you were previously enrolled in a nursing program, please submit a letter including the type of program, name of the school, school address, dates enrolled, and reason for leaving.  Admission of students, who have not been successful in other nursing programs, will be at the discretion of the CSUSM School of Nursing Director.
