Service Learning Forms  

The Office of Community Service Learning (OCSL) requires that all students involved in a service learning class fill out and turn in their forms:

1) Students need to submit their Student Service Learning Plan.

2) Download and print the Student Participation Guidelines. This requires the community partner to review with the student and sign.

3) Students are encouraged use Timesheets to keep track of the hours they serve at the agency.
