Service Learning Forms
The Office of Community Service Learning (OCSL) requires that all students involved in a service learning class fill out and turn in their forms:
1) Students need to submit an on-line registration.
2) Submit a the Guidelines & Limitations form. Download it here. This requires the community partner to review with the student and sign.
3) Timesheets are available here.
