Student Created Video Award Nomination

Nominations are due by 5:00 p.m. on April 9, 2013


Student Created Video Eligibility for the Outstanding Service Learning Video Award:
  • Nominees must have participated in a Service Learning course in Summer 2012, Fall 2012 and/or Spring 2013
  • Nominees must have served hours in Summer 2012, Fall 2012 and/or Spring 2013
  • The service cannot be part of a observation or teaching requirement for students


Please Note:

Films of up to 5 minutes in length may be submitted for consideration for this award.  Films should be appropriately labeled and must include credits.  The preferred format is DVD, and all DVDs must be submitted to the OCSL by April 9, 2013 for full consideration.  If your film is in an alternate format, please contact the OCSL.

We welcome all types of films that are related to community service learning.  This might be an individual effort, or could include a collaboration among students, community organizations, and/or community members served.  The film could also be the end result of the community service project itself (such as a public service announcement a student or group makes to help an organization deliver a message).  It can consist of interviews of various community service learning students, reflecting on their experiences etc., etc.  Be creative – we look forward to receiving your submission!

The Office of Community Service Learning at CSUSM is hereby granted the right to utilize an excerpt from any film submitted for promotional purposes.  The individual, group, or organization submitting the film hereby warrants that it is authorized to commit the film for screening, and understands and accepts these requirements and regulations.  All films submitted are automatically part of the OCSL’s non-commercial film library unless otherwise stated by the filmmaker or representative. The submitter shall indemnify and hold harmless the OCSL at CSUSM from and against any and all claims, liabilities, losses, damages, and expenses (including but not limited to attorney's fees and costs of the court) which may be incurred by reason of any claim involving copyright, trademark, credits, publicity, screening, and loss of or damage to the films entered.

Nomination Final Checklist: 
In order for your nomination to be accepted, make sure you:
  1. Complete each question on nomination the form
  2. Answer all questions for the appropriate nomination
  3. If available, scan attachments such as pictures, newspaper clippings, etc. onto 1 or 2 sheet(s) of paper and e-mail them to ocsl@csusm.edu
  4. Submit nominations by 5:00 p.m. on April 9, 2013

All completed nomination forms are the property of The Community Service Learning Office (OCSL) and will be used at the OCSL's discretion as employees of OCSL at CSU San Marcos.
