Necessary Forms for Students 

The Office of Community Service Learning (OCSL) requires that all students involved in a service learning class fill out and turn in three forms:

1) Click here to submit your on-line registration. THIS FORM IS REQUIRED. This lets the OCSL staff know that you're taking a service learning class.

2) Click here to download the Guide & Limitations form. THIS FORM IS REQUIRED. For this form you're required to obtain three signatures. Signatures needed are from you, your site supervisor and your professor.

3) Click here to download the Realease Waiver. THIS FORM IS REQUIRED.  All students are required to submit this form.

4) Click here to download the Student Evaluation of the Field Placement Agency.  Please submit to OCSL after your service learning has been completed.

5) Click here if you need time sheets.  These time sheets are for your use only. Submit to your Professor if needed; the OCSL does not require them.

To download the forms, simply click on each link above. The forms are in .PDF format and require Adobe Reader.  You cannot fill these forms on-line at this time.

NOTE: Make sure you, your instructor and site supervisor sign the Guidelines and Limitations form (see step #2).  Please leave a signed copy of the Guidelines and Limitations with your site supervisor and return original signed documents to the Office of Community Service Learning to Craven Hall 5211 prior to beginning your service.  Thanks for your co-operation.
