- (Long FAF) 4 Pages of Faculty Assignment Form (For use when assigning 11 or more faculty at once)

Form Directions
ADD a New Section:- All the time: Complete a (SAC) Section Add/Cancel Form – sections A, B, C, & D
- After Schedule is Posted: Please make sure the Dean’s Office has approved adding any new sections (include the approval in the email trail to acdsched)
- For Orientations the Dean’s Office will handle the opening of new sections. There is no need to send forms.
DELETE a Section:
- Can only be done before the schedule is posted (after, it’s a CANCEL): Complete a (SAC) Section Add/Cancel Form – sections A, C, D, & F
CANCEL a Section:
- Only done after the schedule is posted (before, it’s a DELETE): Complete a (SAC) Section Add/Cancel Form – sections A, C, D, & F
CHANGE the Time or Day for a Section:
- Before the Schedule is posted: Use the Change the Time/Day or Enrollment Cap Form
- After the Schedule is posted: Complete a (SAC) Section Add/Cancel Form to Cancel the Section & another Section Add/Cancel (SAC) Form to create the new Section with the new time/day.
Make a Section ACTIVE:
- All the time: Use a Enrollment Control Form
- After Schedule is Posted: Please make sure the Dean’s Office has approved opening any new sections (include the approval in the email trail to acdsched)

Make a Section TENTATIVE:
- All the time: Use a Enrollment Control Form
Change the ENROLLMENT Number:
Change the ROOM:
- All the time: Please submit the Room Change Request Form
ASSIGN Faculty:
- All the time: Use a (FAF) Faculty Assignment Form - input class information & instructor information (include her/his EmpID)
- If assigning more than 11 faculty at once, please use the (Long FAF) 4 Pages of Faculty Assignment Form
UNASSIGN Faculty:
- All the time: Use a (FAF) Faculty Assignment Form - put faculty last name in "Current Instructor" box & STAFF in "New Instructor" box.
Assign MORE THAN ONE Faculty to a single section:
- All the time: Use a (FAF) Faculty Assignment Form - indicate percent responsibility in the "% Resp" box
- Team Taught is 50/50 - each instructor gets 50%
- Overseeing faculty (who are not teaching) - get NI ("Not Included")
