PS Form 2976 is required on all international mailing for items weighing 16 oz or more and all dutiable items regardless of their weight. (Supplies available at the Mail Center)

Please fill out the Return Receipt as seen below.  You may type, use labels or print clearly in blue or black ink. Remember to date and sign both sides of the form.

Place Customs form on the front of you item to be mailed.
