Student Field Placement Procedure
 

Definition


  
Student Field Placement encompasses a variety of opportunities, such as, internships,
   service learning, observations, field work, Senior Experience and volunteer work.
 

Documents

     In an effort to provide a safe and healthy learning environment for students participating in a
     student field placement program, the California State University San Marcos utilizes the follow
     documents:

                   Cal State San Marcos Guidelines and Limitations for Student Field Placement
                   Office of Community Service Learning Student Registration Form


A.  University-Community Partnership Agreement (university contract)

This university contract is a partnership agreement for a community organization to be able to accept Cal State San Marcos student placements (internships, service learning, or other field experiences).  This agreement details the responsibilities of the university and of the organization.  Unless otherwise approved by the Director of Procurement & Support Services, no other agreement format is to be used between the university and an organization accepting student field placements.  This contract must be on file before any student does any
curricula-linked activity at any community
organization.

B.  Guidelines and Limitations for Student Field Placement (student contract)

The student accepts responsibility for his/her actions pursuant to these guidelines
and limitations.  It is a contract amongst a specific student, instructor, and
community supervisor, with regard to a specific community placement experience.

            C.  Office of Community Service Learning (OCSL) Student Registration Form (student information)

This form provides information about the student, the organization, field placement responsibilities, emergency contact information, and meets university data collection responsibilities.

Responsibilities


 Individual and departmental responsibilities are:

   
A.   Faculty - Once a student field placement opportunity is developed, the faculty member is to 
          provide their student(s) with:         

  
     1.  Guidelines and Limitations for Student Field Placement to be read and signed by the
             student, the community site supervisor, and the instructor.  Copies may be made and
             retained by all parties.
The original is to be sent to Procurement & Support
             Services.
 
   
       
2.  OCSL Student Registration Form to be completed. It is the instructor’s responsibility to
             collect these forms from the students.  Copies may be made and retained by all parties. 
             The original is to be sent to
the Office of Community Service Learning.

      B.  Student
 
         1.   Each student is responsible for completing the required forms and obtaining the
              necessary signatures.
 
         2.   It is the student’s responsibility to ensure that these completed and signed forms are  
               returned to their instructor and copies are given to the site supervisor:

       C.  Procurement & Support Services

             Procurement & Support Services, manages the partnership (contractual) agreements
             between the university and organizations accepting student field placements.

              Procurement & Support Services will respond to requests from faculty or staff to establish   
              new organizational contracts by:
 
            1.  Using the Student Field Placement Agreement Database to determine if the organization
                 has a valid agreement on file.

            2.  If an agreement is not on file, Procurement & Support Services staff will prepare and mail the
                 University
-Community Partnership Agreement to the potential field placement site.

            3.  Following up on any agreements not returned by the organization within fifteen (15) business
                 days.

            4.  Signing off acceptance of the agreement and distribute copies to the organization, the
                 appropriate college staff and the university contract file.

            5.  Entering and maintaining the agreement information in the Student Field Placement Agreement
                 database.

            6.  Responding to inquiries regarding contractual issues only.  Procurement & Support Services will
                 refer any inquiries concerning student field placement issues to the Office of Community
                 Service Learning or other appropriate college staff.

           
7.  Verifying that the organizations listed on the Guidelines and Limitations for Student Field
                 Placement
form have a valid agreement on file
. Once this has been verified, Procurement &
                 Support Services, will forward this form to Business Services, Safety and Risk Management. 

    
 D.  Office of Community Service Learning

           Office of Community Service Learning will receive and retain the OCSL Student 
           Registration Form
, and keep campus data on field placement students.

      E.  Business Services, Safety and Risk Management 

            Business Services, Safety and Risk Management will be the final depository for the
            Guidelines and Limitations for Student Field Placement
forms.  These documents will be
            kept on file for a maximum of one-year if there is no report of an incident.

       F.  Environmental Health & Safety

             Any injuries or accidents to students are to be reported by the student, faculty and/or
             community organization to the Associate Director of Environmental Health & Safety,
             ext. 4510, for investigation.
