Defensive Driving Forms

To receive your certification, after you register for your class, you are required to turn in three documents. A copy of your driver's license, the STD 261 form, and the INF 1101 form.

STD 261 Form

The STD 261 Form must be completed prior to using a state vehicle or driving personal vehicles for university business. This form is needed for any employee that chooses to claim mileage while driving a privately owned vehicle. Department managers are responsible for updating this form with their employees each fiscal year and maintaining them in their department. A copy of the STD 261 form must be given to the Risk Management & Safety office.

DMV Pull Notice Program (INF 1101)

All employees who drive on university business are required to be enrolled in the DMV Pull Notice Program. It is important that the campus receives an initial DMV report to ensure that there are not any prior driving violations on the employee's driving record. The DMV will notify the campus of any reported violations through a pull notice report. The DMV will also provide the University an annual report for all other drivers that have maintained a clean record since their last report, based on their anniversary date of enrollment in the DMV Pull Notice Program. All employees must submit a signed (INF 1101) form to the Defensive Driving Coordinator on the day of their scheduled course.

Forms for Car Accidents

State Driver Accident Review Form

The STD 274 Form must be completed by the employee's supervisor within five (5) days from the date of an accident and a copy forwarded to ORIM (Office of Risk and Insurance Management). The form is required for ALL accidents regardless of who was hurt, what property was damaged, or who was responsible.

Vehicle Accident Reporting Form

If an employee is in an accident the employee must fill out the STD 270 Form  within 48 hours of the accident.
