REQUESTING FOREIGN TRAVEL INSURANCE
EMPLOYEE INSTRUCTIONS
Step 1 READ: EMPLOYEE Guidelines
Step 2 VERIFY HIGH HAZARD STATUS: If yes, requires Chancellor's Office Approval First
High Hazard List (As determined by the Chancellors Office & Insurance Carrier)
State Department Travel Warnings
Step 3 ENROLL: Safe Traveler Enrollment Program (STEP)
Step 4 COMPLETE: Foreign Travel Insurance Request Form
Step 5 SUBMIT COMPLETE PACKAGE TO RM&S:
- Completed Foreign Travel Insurance Request Form
- Green Travel Authorization Form (or equivalent) signed by approving authority
- Proof of enrollment in STEP program (print screen is acceptable)
- Verification of High Hazard Status (State Department Travel Warning & CSU High Hazard List). Travel to High Hazard areas require Chancellors Office approval first before insurance can be procured.
