All work-related injuries must be reported immediately to all of the following:

Your Supervisor
University Police (x4567)
Risk Management & Safety (x4502)

Worker's Compensation Program

Injured employees must receive a Workers' Compensation claim form within 24-hours, please call Janice Plemons x 4502 to request a claim form.

HOLIDAY NOTIFICATION - Please be advised that our industrial medical provider (Concentra) located on Nordahl Road) will be closed during holidays.   Employees working those days (and any other holidays) are directed to call the clinic, and have doctor paged, at (760) 432-900 prior to going to Palomar Hospital's Emergency Room. Our industrial doctor will meet the injured employee at the hospital. This is to minimize time spent in the Emergency Room by not having to wait for the attending ER doctor.
