Patient's Rights and Responsibilities


As a Student Health and Counseling patient, You have the Right to:
  1. be treated with respect, consideration and dignity
  2. be given privacy during discussion, examination, and treatment
  3. have all communications and records pertaining to your health care treated as confidential
  4. be provided information regarding your diagnosis, treatment, and prognosis in language you understand
  5. approve or refuse the release of information regarding your health care
  6. receive reasonable continuity of care and know the names and title of those participating in your health care
  7. participate in decisions involving your care 
  8. refuse treatment at any time
  9. be informed of SHCS' policies regarding your rights and responsibilities and availability of services
  10. use SHCS channels to effectively express concerns or suggestions about the clinic
  11. change primary providers if other qualified  providers are available.

As a Student Health and Counseling patient, You are Responsible for:

  1. showing respect to staff and other patients
  2. arriving on time for appointments and canceling appointments ahead of time if you cannot make them
  3. knowing the name of your health care provider
  4. providing information needed to assure your proper evaluation and treatment
  5. asking questions about your care
  6. understanding the prescribed treatment
  7. giving feedback about clinic policies and quality of care
  8. participating fully in your health care
