Mentor Application

Please be sure to review the FAQs or the website for information about the program and eligibility. If accepted into the program, you must be available for the following dates:

Welcome BBQ - Wednesday, August 31, 2016; 4:00PM-7:00PM
Meetings/Workshops - Wednesdays; 4:00PM-5:00PM (9/7, 9/14, 9/28, 10/19, 11/16)
Semester-End Celebration - Wednesday, December 7, 2016; 4:00PM-6:00PM

Attendance is a necessary part of being in the Peer Mentoring program. You will be notified by early-July about status in the program.

Applications are due by 11:59PM on Thursday, June 30, 2016. Please note that * indicates a required response. Incomplete responses will prevent submission of the application.
