Spring 2013 CSUSM Student Organization Recognition Form

Prior to completing this form, ensure that Presidents and Treasurers meet CSU minimum requirements.  Minor student officers must maintain a minimum 2.0  GPA each term, be in good standing with the University, be enrolled in 6 semester units (3 units for graduate students), and have earned a maximum of 150 semester units (50 graduate level units).

To complete the recognition process, you must complete this on-line form, attend SORW, and submit the following to SLL:

1. Recognition Agreement Form

2. ASI Liability Agreement Form

3. A current membership list including campus IDs and campus emails (all organizations must have at least 5 CSUSM student members)

4. A current Constitution

Templates of both the constitution (with CSU-updates for 2012-2013) and the membership roster will be emailed to all eligible presidents upon verification of officer eligiblity.

I understand that submitting this online form does not complete the recognition process. This organization is now responsible for submitting an Officer Agreement form, ASI Bank Form, and Membership Roster by 12/4. A current constitution will be submitted at the start of the Spring 2013 semester. SLL will verify the Advisor's role for the Spring13 semester and both President and Treasurer must attend SORW on 12/6 from 8-12 located in UHAL 441. to complete the CSUSM Student Organization Spring 13 recognition requirements.
