2014-2015 CSUSM Student Organization Online Recognition Form

Prior to completing this form, ensure that Presidents and Treasurers meet CSU minimum requirements.  Minor student officers must maintain a minimum 2.0  GPA each term, be in good standing with the University, be enrolled in 6 semester units (3 units for graduate students), and have earned a maximum of 150 semester units (50 graduate level units).

To complete the university recognition process, you must complete this on-line form, attend SOLC, and submit the following to SLL:

1. Officer Agreement Form

2. A current membership roster including campus IDs and campus emails (all organizations must have at least 5 CSUSM student members)

3. A current Constitution

All organizations must use the provided templates to submit the constitution and the membership roster for the 2014-2015 Acdemic Year.

I understand that submitting this online form does not complete the recognition process. This organization is now responsible for submitting an Officer Agreement form, Membership Roster, and a EO 1068 compliant Constitution by August 1st. SLL will verify the Advisor role and both President and Treasureror the 2014 -2015 Academic Year. Both student officers must attend SOLC on 8/22/14 to complete the CSUSM Student Organization recognition requirements.
