CSUSM Corporation Open Enrollment for 2019

CSUSM Corporation's Open Enrollment period is open until November 8th, 2018.

You must submit all required documentation to no later than 5pm on November 8th, 2018.

If you are interested in making changes to your benefits package, including health insurance plan, vision plan, and dental plan, please review the benefit summaries below.  All changes will be effective January 1st, 2019. 

If you do not want to make any changes to your current insurance enrollment, you DO NOT need to complete any form or submit any information to CSUSM Corporation or hrcorp@csusm. Your coverage will automatically continue as it currently is, with any plan changes as noted on the CSUSM Corp 2018 vs 2019 Benefit Comparison.
However, if you currently receive cash in lieu of benefits, you MUST complete the 2019 Benefits for Cash Enrollment Form.

Required Documents

Please complete and return the following required forms to make changes to your existing benefits enrollments:

  1. CSUSM Corp - Medical Benefits Participant Enrollment Form (use this form for both Kaiser and Anthem)
  2. Delta Dental Enrollment Form
  3. VSP Enrollment Form

Once these forms are complete, please scan them to , or drop them off in the CSUSM Corporation office, attn: Human Resources in Craven 5111. 

If receiving cash in lieu of benefits, please complete and return the following form for 2019:

To find out if your doctor is in the Anthem Blue Cross network, or to find a new doctor or pharmacy in the Anthem Blue Cross network, go to the Find a Doctor tool. If you choose to enroll in either of the Anthem HMO plans, you will need to provide the Primary Care Physician Code on the enrollment form for each person covered under your plan.
