Form Submission Deadline is 4:00 p.m. on Friday, October 15th.
The Benefit Enrollment Worksheet should be submitted to firstname.lastname@example.org.
The 2022 Dependent Care_Health Care Reimbursement Form is processed/submitted through AdobeSign.
ANNUAL BENEFITS INFORMATION & ENROLLMENT
Please review the 2021 Open Enrollment Benefits Guide Booklet, the information below, as well as the links of information and forms located on
the right side bar.
There is nothing required to do, if you do not wish to make any changes to your current
medical benefits. Your current elections for these plans will automatically continue
for the 2022 Plan Year. However, you must re-enroll every year in a Health Care and /or Dependent Care Reimbursement
Account (HCRA and/or DCRA).
You are able to enroll, change or cancel the benefit plans listed below:
- CalPERS Health
- Delta Dental
- Vision Service Plan (VSP) Premier Vision Plan
(enrollment & cancellation for the Premier Plan may only be done during Open Enrollment
and after the completion of 12 continuous months of enrollment)
- CSU FlexCash (in lieu of health and/or dental)
- MetLaw Legal Plan
- The Standard Insurance

Note: Enrollment in any of the above benefit plans are restricted to Open Enrollment only, unless, a qualified event mid-year has occurred.

Effective January 1, 2022 rates for most health plans will change. Refer to the 2022 Health Premium Rate Sheet.

- You will receive new health plan ID cards if you change your health plan or enroll
for the first time.
- If you change plans during Open Enrollment and you do not see the correct deduction
applied by your February pay warrant, contact the HR Benefits Office at email@example.com
- If you change health plans, do not continue to use your previous health plan after
December 31, 2021.

Delta Dental PPO Plan Enhancement Reminders
- Diagnostic and Preventative services (ie. cleanings and exams) will not count against
your annual maximum. This benefit does not apply if you visit a Delta Dental Premier
or non-Delta Dental dentist.
- SmileWay Wellness Benefit is an enhanced coverage for higher risk employees. Employees
with certain qualifying diagnosed medical conditions (diabetes, heart disease, HIV/AIDS,
rheumatoid arthritis or stroke) are offered expanded coverage. In order to use this
great enhancement, you will need to "Opt In" to the SmileWay program. Information
on how to "Opt In" can be found on the flyer's below:
Value-Added Features from Delta Dental
- Hearing Aid Discounts - Amplifon Hearing Health Care offers discounts on top hearing

Vision plans are offered through Vision Service Plan (VSP). The CSU provides Basic
vision insurance automatically for eligible employees and their families at no cost
to the employee. During Open Enrollment, employees have the option to upgrade to
an enhanced plan (VSP Premier Plan) for a small monthly cost.

to have vision coverage, otherwise they will lose their coverage.)
Flexible Spending Account Reminder
All Employees must complete the 2022 Dependent Care/Health Care Reimbursement Account Plans Enrollment Authorization
form to enroll in either program for 2022, regardless of participation in 2021. The maximum
annual contributions remain the same for both plans (HCRA -$2,750; DCRA - $5,000).
As well, over-the-counter medications will no longer need a prescription, per the
CARES Act with a retroactive date of January 1, 2020.
All first time HCRA participants will be issued 2 ASIFlex Visa Debit Cards, which
can be used to pay for qualifying expenses. There is no cost for the cards and applications
are no longer required.
Employees who currently have a valid ASIFlex Card may continue to use their current
card until it expires.
