Employee is to complete and return the WC forms (DWC-1 & Intake Sheet) to the WC Coordinator within 24 hours via email, fax, or in-person.
Manager is to complete and return the “Initial Investigation Report” (Form IIPP-5) to the WC Coordinator by the end of the work day on which the injury occurred, if possible, and within 24 hours by law.
