Calendar of Important Dates for Annual Staff Performance Evaluation Cycle

Performance Evaluation Forms for Represented Employees

  • Physicians - Unit 1
  • California State University Employees' Union (CSUEU) - Units 2, 5, 7 & 9: A draft of the performance evaluation must be provided to the employee to provide input for consideration prior to the final evaluation being completed. The employee then has 5 workdays to submit his/her input to the draft evaluation. The employee may request up to 5 additional workdays to complete their review of the draft and provide input prior to the final evaluation being completed.
  • Academic Professionals of California (APC) Unit 4: A draft of the performance evaluation must be provided to the employee to provide input for consideration. The employee then has 14 calendar days to submit a rebuttal prior to the final evaluation being completed.
  • State Employees Trades Council (SETC) Unit 6
  • Statewide University Police Association (SUPA) Unit 8
  • Temporary Employees: Long term (more than 180 days) should receive an evaluation each year at the time of the annual schedule.

Annual Evaluation Period: July 1, 2016 - June 30, 2017; Due to OHR: August 18, 2017

The annual evaluation cycle for Vice Presidents, Provost and Deans is September 1 through August 31.
