Calendar of Important Dates for Annual Staff Performance Evaluation Cycle

Performance Evaluation Forms for Represented Employees

  • Physicians - Unit 1
  • California State University Employees' Union (CSUEU) - Units 2, 5, 7 & 9: A draft of the performance evaluation must be provided to the employee to provide input for consideration prior to the final evaluation being completed. The employee then has 10 workdays to submit his/her input to the draft evaluation. 
  • Academic Professionals of California (APC) - Unit 4: A draft of the performance evaluation must be provided to the employee to provide input for consideration. The employee then has 14 calendar days to submit a rebuttal prior to the final evaluation being completed.
  • Teamsters Local 2010 - Unit 6
  • Statewide University Police Association (SUPA) - Unit 8
  • Temporary Employees: Long term (more than 180 days) should receive an evaluation each year at the time of the annual schedule.
