Independent Study Request Form

This form should only be used to create independent study courses from the Registrar's Office, CEHHS, and COBA. This form is to ONLY be filled out by Registrar Staff, Academic Coordinators, or Department Chairpersons (This is NOT a student form).

Student forms are available under these pages, depending what college the Independent Study Course is in: CHABSS, CSTEM, COBA, and see advising for CEHHS. For online submission, please refer to the Prerequisite Waivers, Enrollment Issues & Independent Study page.

Please see the Schedule Forms page for directions on how to make other schedule changes. The submitter's email listed below will be sent a confirmation email after submission.
