
COVID-19 Vaccination Requirement

The California State University is requiring that all students must be fully immunized (to include required booster, when eligible) against SARS-CoV-2, the virus that causes COVID-19. Interim Policy is available on the CSU website.
- Students must submit their COVID-19 vaccination record(s) or have an approved medical accommodation/exemption or religious exemption on file BY DUE DATE: March 15, 2023. Please see instructions below for submission.

Medical or Religious accommodation/exemption | Student residents may seek an accommodation/exemption from the COVID-19 vaccine requirement
for medical or religious belief.
Medical accommodation/exemption: Please submit an accommodation/exemption include documentation from your licensed physician for a medical accommodation/exemption. Religious accommodation/exemption: Please submit an accommodation/exemption ASAP include a personal statement for the religious accommodation/exemption. |
How to Submit COVID-19 Documentation
- For COVID-19 vaccination records and medical exemption documentation: use the SHCS Immunization Submissions form.
- For religious accommodations/exemptions: use the Religious Exemption form (for COVID-19 vaccination only). Please note that you must include a personal statement.
Authorize Request from California Immunization Registry
You may instead choose to authorize CSUSM Student Health & Counseling Services to request your COVID-19 immunization data from the California Immunization Registry (CAIR). The requested data will be used strictly to determine compliance with the COVID-19 vaccination requirement. To do so, log into MyCSUSM and click on ‘I consent’ in the Immunization pop-up message.
Please note: The California Immunization Registry includes data submitted by healthcare providers in the state of California. Your healthcare provider may or may not have submitted your immunization records. If your records are not available, you will be required to submit your records directly to CSUSM.
