The California State University is requiring that those accessing campus facilities this fall be immunized against SARS-CoV-2, the virus that causes COVID-19. Interim Policy is available on the CSU website.
Students are encouraged to submit their COVID-19 vaccination record or have an approved
accommodation/exemption on file by August 16, 2021. Please see instructions below
Antibody tests will NOT be accepted as compliance.
Students have until September 30th, to upload their vaccination record or have an approved exemption on file. Until that final deadline of Sept. 30th, student who have not submitted proof of vaccination will be required to participate in mandatory weekly testing program. Students who fail to comply with the policy may be subject to Student Conduct disciplinary actions.
Please note that students living in UVA or The Quad are required to submit their vaccination record by Aug. 16, per the License Agreement, in order to move into university housing. This date is different from the systemwide requirement.

|Medical or Religious Accommodation||
Student residents may seek an accommodation from the COVID-19 vaccine requirement for medical or religious belief.
Please submit the self certification in MyCSUSM and be ready to include documentation from your licensed physician for a medical accommodation
or personal statement for the religious accommodation. Students choosing to submit
either accommodation and are approved, will be required to continue weekly testing
and masking protocols when on campus.
Step 1: Log into MyCSUSM and click on COVID 19 Self-Certification under Quick Links
Step 2: Select the appropriate certification, as applicable and click Next
Step 3: If certifying that you are vaccinated or qualify for a religious or medical exemption,
upload the necessary supporting documentation on the next page.
