Effective Date of Coverage - Health

Eligible employees must enroll in a medical plan within sixty (60) days of employment, or reemployment if there has been a break in service of more than thirty (30) days.

The standard effective date for medical coverage is the first of the month following the permitting event and receipt of signed enrollment form, including any required supporting documentation to the Office of Human Resources.  Retroactive deductions may occur if the deduction is not processed prior to the effective date of enrollment, and may require an Accounts Receivable (A/R) to be set up for payment.

Employees who fail to enroll within the sixty (60) day timeframe will delay the effective date of medical coverage for a minimum of ninety (90) days after submitting the appropriate documents to the Office of Human Resources or can opt to enroll during the next open enrollment period. Reenrollment or change in enrollment follow the same effective dates as enrollment.

Some change in status events require a mandatory effective date of coverage, which is effective the first of the month following the permitting event date, regardless of when the signed medical enrollment document is submitted to the Office of Human Resources.

An employee who is on leave of absence without pay, temporary disability compensation, non-industrial disability leave, or other non-pay status may not enroll while in such status. However, the employee may enroll within sixty (60) days of returning to active status provided that he/she would otherwise have been eligible to enroll during the non-pay status.

Eligible employees who are granted a leave of absence without pay may continue their participation in their medical plan by making direct payments of the full premiums to the plan provider.
