Immunization Requirement

Required Immunizations for New Students 

The California State University (CSU) is committed to protecting the health and well-being of our students, campus, and community. To comply with this overarching goal, CSU campuses implement procedures to ensure that students are educated about and receive immunizations to prevent potentially serious and contagious diseases. All new students are required to provide proof of vaccination or immunity to the following prior to the start of their first semester at CSUSM.

  • Hepatitis B (Hep B)
    • Three (3) doses OR laboratory evidence of immunity (positive titer).
    • Only required for students 18 years and younger at the start of their first semester at CSUSM.
    • Both Hepatitis B vaccine and titer lab test available for a small fee at SHCS.
  • Measles, Mumps, Rubella (MMR)
    • Two (2) doses OR laboratory evidence of immunity (positive titer).
    • First dose must be on or after 1st birthday.
    • Required for ALL students.
    • Both MMR vaccine and titer lab tests available for a small fee at SHCS.
  • Meningococcal Conjugate (MenACWY)
    • One (1) dose given within last four years.
    • Only required for first-time residential students (living in university or affiliated housing), who are 21 years and younger.

An immunization hold is placed on accounts that are noncompliant after the deadline (~6th week of the semester). Once the hold is placed, students are unable to register for the following semester until the requirement is met.

Submit Immunization Records

Instructions: 

  • Full Vaccination or Titer: Upload a copy of your immunization records or lab report.

  • Medical Exemption: A student with a medical condition that prevents them from being safely vaccinated may be exempted from the requirement. Please complete the Medical Exemption Form with documentation and signature from a licensed physician (MD,DO), physician assistant (PA) or nurse practitioner (NP).

  • Religious/Strongly Held Personal Belief Exemption: A student may be exempted from receiving a required immunization due to either (i) a person's sincerely held religious belief, observance, or practice, which includes any traditionally recognized religion, or (ii) beliefs, observances, or practices which an individual sincerely holds and that occupy a place of importance in that individual's life, comparable to that of traditionally recognized religions. Please complete the Religious Exemption Form.

CSUSM students are strongly encouraged, but not required, to receive the following immunizations: 

  • Varicella (Chickenpox)
  • Tetanus-Diphtheria-Pertussis (Tdap)
  • Meningococcal Conjugate (MenACWY)
  • Tuberculosis Screen/Risk Assessment
  • Hepatitis A (Hep A)
  • Human Papillomavirus (HPV)
  • Influenza (Flu)
  • Meningococcal B (Meningitis B)
  • Pneumococcal
  • Poliovirus (Polio)
  • COVID-19 Vaccination
