Ergonomic Evaluation (Campus Step 2)

After completing Campus Step 1, employees may request an in-person evaluation of their work space by filling out the following Ergonomic Evaluation Request Form:

A report will be provided to the employee within 30 days of the evaluation. Employees are encouraged to review the report information with their supervisor. Some suggestions may involve the use of specific ergonomic equipment (Campus Step 3) or reconfiguring the workstation (Campus Step 4), or both.
