Arts Facilities Request Form

An email confirming your reservation will be sent to the email address
listed below within 48 hours. You will also be notified if a Request
For Facilities Use (RFU) form will be required prior to your reservation
being confirmed. Students can obtain a RFU at the Student Life and
Leadership (SLL) Office in Administrative 3400.

You will be asked to sign in to your Microsoft Online account. Please use your campus credentials, otherwise you will not be able to access the form.
