Requesting Foreign Travel Insurace for Employees

REQUESTING FOREIGN TRAVEL INSURANCE

EMPLOYEE INSTRUCTIONS

Step 1-CHECK: Please check with your division/department FIRST to make sure you're following required protocols

Step 2-READ: EMPLOYEE Guidelines

Step 3-VERIFY HAZARD LEVEL: May Require Chancellor's Office Approval

      War Risk List (As designated by the Chancellor's Office & Insurance Carrier)
      High Hazard List (As designated by the Chancellor's Office & Insurance Carrier)
      State Department Travel Warnings

Step 4-ENROLL:

      Safe Traveler Enrollment Program (STEP) (Available only to U.S. Citizens)

Step 5-COMPLETE:

      Foreign Travel Insurance Request Form  
      Instructions For Foreign Travel Insurance Request Form

Step 6-SUBMIT COMPLETE PACKAGE TO SR&S:
  • Completed Foreign Travel Insurance Request Form
  • Green Travel Authorization Form (or equivalent) signed by approving authority
  • Proof of enrollment in STEP program (print screen is acceptable) (Available only to U.S. Citizens)
  • Verification of High Hazard Status (State Department Travel Warning & CSU High Hazard List). Travel to War Risk areas require Chancellor's Office approval first before insurance can be procured.
