Student Travel

The requirements and documentation for students traveling under the Division of Academic Affairs may vary depending on the program that is funding the travel.

In addition to following the process below, students should work closely with the sponsoring unit to insure that submission deadlines are met and their package is complete.

The sponsoring unit is responsible for following AA Division protocols and submitting the travel request packet for the Provost’s approval.

Domestic Travel - Student

Upon approval by college/unit, submit travel package to Provost in the following order:

  1. Provost Transmittal Form
  2. Request for Travel Approval Form or UARSC Travel Request
  3. Provost Student Air-Ground Travel Authorization Form
  4. Release of Liability Form
  5. Student Conduct Agreement
  6. Documents supporting travel request

International Travel - Student

Please refer to Academic Affairs International Travel Protocol before beginning your travel package.  Upon approval by college/unit, submit travel package to Provost in the following order:

  1. Provost Transmittal Form
  2. Request for Travel Approval Form or UARSC Travel Request
  3. State Department Warning List
  4. High Hazard Warning List
  5. CSUSM High Hazard/Warning List Travel Request Form (if travel destination(s) are listed on either item #3 or #4 above)
  6. Safe Traveler Enrollment Program (STEP)  (submit a screen shot of verification of trip enrollment)
  7. Foreign Travel Insurance Insurance Request Form
  8. Provost Student Air-Ground Travel Authorization Form
  9. Release of Liability Form
  10. Student Conduct Agreement
  11. Documents supporting travel request
