Enrollment in vision is automatic and is effective the first of the month following appointment. Reenrollment or change in enrollment follow the same effective dates as enrollment.
An employee who is on leave of absence wtihout pay, temporary disability compensation, non-industrial disability leave, or other non-pay status may not enroll while in such status. However, the employee may enroll within sixty (60) days of returning to active status provided that he/she would otherwise have been eligible to enroll during the non-pay status.
Eligible employees who are granted a leave of absence without pay may continue their participation in their vision plan by making direct payments of the full premiums to the plan provider.
