If you would like to inititate a Community Partnership Agreement with an organization that is not already listed in the Office of Community Service Learning Database or in the attached General Database, please complete the following form and click the submit button.

The completed form will automatically be sent to Procurement & Support Services and you will be notified when your request has been processed.  Once the Community Partnership Agreement is fully executed, a copy will be e-mailed to you.
