Event Evaluation

The purpose of the Post Event Evaluation is to reflect upon your event to see how it fits into the mission of ASI.

This form must be submitted no more than ten (10) days following the event along with at least three (3) event photos.

When possible, please submit photos with a resolution of no less than 300ppi. All photos must be in JPG file format.
