Reinstatement Petition Form

Students who have been academically disqualified from the university may petition for reinstatement.

Important: If you have been away from the university for two or more semesters, you must reapply as a transfer student.

Readmitted Students: Out one semester

If you have been readmitted to the university after being away for one semester, please contact Monica Huerta for questions or assistance.

Readmitted Students: Out two or more semesters

If you have been readmitted to the university after being away for two or more semesters, please contact Alexa Diaz, for questions and guidance.
