REQUESTING FOREIGN TRAVEL INSURANCE

STUDENT INSTRUCTIONS

Step 1-READ: STUDENT Guidelines

Step 2-VERIFY HIGH HAZARD LEVEL: May require Chancellor's Office Approval
   High Hazard and War Risk Lists (As designated by the Chancellor's Office & Insurance Carrier)
   State Department Travel Warnings

Step 3-ENROLL:

   Safe Traveler Enrollment Program (STEP) (Available only to U.S. Citizens)

Step 4-COMPLETE:

   Foreign Travel Insurance Request Form  
   Instructions for Foreign Travel Insurance Request Form

Step 5-SUBMIT COMPLETE PACKAGE TO SR&S:    

• Completed Foreign Travel Insurance Request Form
• Green Travel Authorization Form (or equivalent) signed by approving authority

• Proof of enrollment in STEP program (print screen is acceptable) (Available only to U.S. Citizens)
• Verification of High Hazard Status (State Department Travel Warning & CSU High Hazard List). Travel to War Risk areas require Chancellor's Office approval first before insurance can be procured.
