
Daily Personal Health Assessment
Before arriving on campus or participating in any campus-related activity or program, all students, employees, vendors and contractors are required to perform a personal health assessment to ensure that they are not experiencing any COVID-like symptoms.
Although COVID-19 symptom self-assessment is required, the online screening submission form is no longer required. By showing up on campus, all employees, students, vendors and contractors are confirming that they are not experiencing any COVID-19 symptoms.
Do not come to campus if:
- You are feeling unwell
- Have any COVID-like symptoms
- Are unvaccinated and have had a known exposure to COVID-19.
Anyone who develops symptoms while on campus or participating in a campus-related activity must immediately do the following:
- leave campus (or the activity location if off-campus)
- advise their manager
- report it to covidcasereporting@csusm.edu
