Absence From Class/ Class Accommodation

Absence from face to face classes for any reason should be reported on the following form, which is housed on the Faculty Affairs website under the Faculty Resources tab labeled Leaves of Absence. 

Absence from Class forms should be routed through Adobe sign with the following signatures: faculty member, department chair, Associate Dean Carmen Nava (cnava@csusm.edu) and have the CHABSS Resource Mailbox (chabss@csusm.edu) as an acceptor.
