- For NAME changes, complete this Name Change Form via the Registrar's Office.
- For your STUDENT EMAIL NAME changes, go into the name settings to change that yourself. The Name Change form does not automatically change this.
- For PRONOUN changes, log into your CSUSM account. Go to "Student Center" then select "Pronouns" in the drop-down menu under "Personal Information" and click Go. Select your pronoun and click SAVE.
- For Graduation/Diploma: Please note that the Commencement Office pulls your name from MyCSUSM for both the Commencement ceremony and your diploma, so make sure it is accurate.
