Questionnaire

Why Use

The Moodle Questionnaire module is a survey-like type of activity. It allows teachers to create a wide range of questions to get student feedback e.g. on a course or activities. You can give points to students for completing the survey without identifying the student response.

How to setup

  1. In your course, select the "Create learning activity" link in the appropriate topic section.
  2. Select Questionnaire from the Activities list.

  3. Add the title and a brief description for the survey.
  4. Add the open date/time and the close date/time.
  5. Configure the settings for the Response options
    1. Type
    2. Respondent Type
    3. Students can view ALL responses
    4. Send submission notifications
    5. Save/Resume answers
    6. Allow branching questions
    7. Auto numbering
    8. Submission grade
  6. Click the Save and display button to add questions

Types of questions

  • Check Boxes
  • Date Box
  • Dropdown choices
  • Essay box - HTML editor possible, set width and height of box
  • Numeric - can set length and number of decimal places
  • Radio buttons - have labels you determine for each question
  • Scale - can customize in many ways
  • Text box
  • Yes/no

Add questions

  1. Click on the "Add questions" link
  2. Select a question type from the drop down menu -> click the add selected question type button
    1. Check Boxes
      1. Add the question name
      2. Choose if the response is required or not
      3. Decide if you want minimum forced or max forced responses
      4. Type the question in the Question Text box
      5. Type the answer choices in the Possible answers box
        NOTE: list one answer choice per row
      6. Select the save changes button
    2. Date Box
      1. Add the question name
      2. Type the question in the Question Text box
      3. Select the save changes button
    3. Dropdown choices
      1. Add the question name
      2. Select if the response is required or not (No is the default)
      3. Type the question in the Question Text box
      4. Type the answer choices in the Possible answers box
        NOTE: list one answer choice per row
      5. Select the save changes button
    4. Essay box - HTML editor possible, set width and height of box
      1. Add the question name
      2. Select if the response is required or not (No is the default)
      3. Leave the Reponse format at the default HTML editor
      4. Input box size -> choose how many lines you want available for the answer
      5. Type the question in the Question Text box
      6. Select the save changes button
    5. Numeric - can set length and number of decimal places. Use if you expect the response to be a correctly formatted number.
      1. Add the question name
      2. Select if the response is required or not (No is the default)
      3. Select the maximum digits allowed
      4. Select the Nb of decimal digits
      5. Type the question in the Question Text box
      6. Select the save changes button
    6. Radio buttons - have labels you determine for each question
      1. Add the question name
      2. Select if the response is required or not (No is the default)
      3. Choose if you want the buttons to be verticle or horizontal aligned
      4. Type the question in the Question Text box
      5. Type the answer choices in the Possible answers box
        NOTE: list one answer choice per row
      6. Select the save changes button
    7. Scale - can customize in many ways
      1. Add the question name
      2. Select if the response is required or not (No is the default)
      3. Select the Nb of scale items
      4. Choose the type of rate scale
      5. Type the question in the Question Text box
      6. Type the answer choices in the Possible answers box
        NOTE: list one answer choice per row
      7. Select the save changes button
    8. Text box
      1. Add the question name
      2. Select if the response is required or not (No is the default)
      3. Set the input box length (20 characters is default)
      4. Set the maximum text length (25 characters is the default)
      5. Type the question in the Question Text box
      6. Select the save changes button
    9. Yes/no
      1. Add the question name
      2. Select if the response is required or not (No is the default)
      3. Type the question in the Question Text box
      4. Select the save changes button

Advanced settings

  1. Choose the Questionnaire Type
    1. Private, Public, or Template
  2. Edit the title (if necessary)
  3. Add any instructions in the Additional Info box
  4. Add a confirmation URL (if needed) to redirect students after the questionnaire is submitted
  5. Add a message for the students after they submit in the Body text box (if needed)
    NOTE: The default message "Thank you for completing this Questionniare" will be displayed if nothing is added.
  6. Add an email if you want a copy sent outside Cougar Courses
  7. Select the save and return to course button

Non-respondents

A list of students who have not completed the survey. You can send a reminder message.

  1. Select the box to the far right of their name.
  2. Type the subject in the Subject box
  3. Type the message in the Message body
  4. Select the Send button

Edit questionnaire settings

  1. Click on the title of the questionnaire
  2. Select the edit settings gauge in the upper right
  3. Select the edit settings link
  4. Make changes as needed
  5. Select the save and return to course button

Edit questions

  1.  Click on the title of the questionnaire
  2. Select the edit setting gauge in the upper right
  3. Select the Questions link

  4. Select the edit gauge icon to the right of the position number for the question that needs edited
    edit gauge to edit question in questionnaire
  5. Make changes
  6. Select the save changes button
