Overview
Advance care planning conversations help patients consider and document the treatment choices they would make if they could no longer speak for themselves. Such conversations are a routine part of healthcare, but guiding patients through them takes skill and a framework of support. This 2-hour course outlines how to integrate advance care planning into your daily work, using best practices to improve patient and practice outcomes.

The updates reflect the latest research, data, and up-to-date information. The learning experience has been enhanced with revamped interactive exercises, engaging videos, and new handouts designed to reinforce key concepts. These updates were made in direct response to feedback from past participants, ensuring the content is more relevant, practical, and impactful than ever before.The refreshed curriculum now includes:
- Practical tools and sample materials to evaluate and enhance advance care planning (ACP) practices
- Current research highlighting the clinical and operational benefits of effective ACP
- Newly added resources to support staff training and patient education
