Eligible employees must enroll in a dental plan within sixty (60) days of employment, or reemployment if there has been a break in service of more than thirty (30) days. New employees who fail to enroll within the sixty (60) day timeframe will delay the effective date of dental coverage for a minimum of ninety (90) days after submission of the appropriate document to the Office of Human Resources, or can opt to enroll during the next open enrollment period.
The standard effective date of dental coverage is the first of the month following the receipt of a signed enrollment document in the Office of Human Resources. Retroactive deductions may occur if the deduction is not processed prior to the start of the effective date of enrollment.
Some change in status events require a mandatory effective date of coverage, which is effective the first of the month following the permitting event date, regardless of when the signed dental enrollment document is submitted to the Office of Human Resources.
An employee who is on leave of absence wtihout pay, temporary disability compensation, non-industrial disability leave, or other non-pay status may not enroll while in such status. However, the employee may enroll within sixty (60) days of returning to active status provided that he/she would otherwise have been eligible to enroll during the non-pay status.
Eligible employees who are granted a leave of absence without pay may continue their participation in their dental plan by making direct payments of the full premiums to the plan provider.
